Family Doctor Finder
Family Doctor Finder can make it easier for you and your family to find a team of healthcare professionals (including family doctors and/or nurse practitioners) as part of home clinic that provides you with a base for your health care needs.


How To Register
Simply register online *or call and they will work with you to connect you with a home clinic in an area that will meet your needs.
To register by phone, please call their contact center between 8:30 a.m. and 4:30 p.m. Monday to Friday at:

Contact Directly By Phone
If you fall under one of the following categories:

Under the age of 16, a group/family of more than 12 people, or feel your registration request may be more complex;

Previously registered with the program, and your application may still be active; or
-
If you do not have a Manitoba Personal Health Information Number (PHIN).


You Will Be Asked
your name, date of birth, mailing address, PHIN (personal health identification number) and telephone number. You will also be asked: your preference for location, language, if you would like a female or male doctor and the type of health care provider you would prefer (family doctor, pediatrician, or nurse practitioner).
