A home clinic is a patient-centered family practice that a patient/client identifies as the place that serves as a home base or central hub for timely care and coordination of all your health and medical needs over time. Research shows that using this model leads to better healthcare, better patient experience and better value for taxpayers.

Family Doctor Finder

Family Doctor Finder can make it easier for you and your family to find a team of healthcare professionals (including family doctors and/or nurse practitioners) as part of home clinic that provides you with a base for your health care needs.

How To Register

Simply register online *or call and they will work with you to connect you with a home clinic in an area that will meet your needs.

To register by phone, please call their contact center between 8:30 a.m. and 4:30 p.m. Monday to Friday at:

204-786-7111 (in Winnipeg) or Toll-free at 1-866-690-8260; TTY/TDD call 774-8618 or Manitoba Relay Services toll-free 1-800-855-0511.

Contact Directly By Phone

If you fall under one of the following categories:

  • Under the age of 16, a group/family of more than 12 people, or feel your registration request may be more complex;

  • Previously registered with the program, and your application may still be active; or

  • If you do not have a Manitoba Personal Health Information Number (PHIN).

You Will Be Asked

your name, date of birth, mailing address, PHIN (personal health identification number) and telephone number. You will also be asked: your preference for location, language, if you would like a female or male doctor and the type of health care provider you would prefer (family doctor, pediatrician, or nurse practitioner).

Once you are registered you will be given a number, and staff in your health region called Primary Care Connectors will call you with the goal of connecting you to a doctor in a location close to you.
